Tracy asked me to post the video of Doc's fall off the DW to see if Olga can see if it was just one of those things or a handler error.

Hmm.. Can't tell why Doc fell off the DW.. I don't see anything that Tracy may have done. Looks like an accident to me.

 Thank you so much for recompiling and posting this video. I appreciate your time. We've recently built a training exercise with jump directly in front of DW, so I don't understand why this happened.Â
Olga, so far, he's behaving normally. I did stop on the way home and bought some arnica, and popped one in his mouth. He seems fine, but my heart sure jumped when he fell. We're just finishing breakfast, and then we're going for a walkie-walk and inspect him during it. (Then I get to take the babyBeast on her walk)...
 Thank goodness we escaped this snafu.

Looks to me like his left hind leg missed the board just before getting to the top ramp. Shadow has done this when she was really hauling butt first run of the day cool weather etc. We saw that she lowered her center of gravity and seemed to get wider wold take a missstep and off she would go. Hope he is feeling better!! (tracy too!!)

Well, you are right, Doc didn't manage to get on the DW square and thus he didn't have enough width of the board to run on.. Why did this happen? No telling..
In general, Doc has not have the foundation of how to handle the DW. He hasn't been taught to seek purchase with all four feet and target the middle of the plank. And like the majority of Agility dogs he also doesn't have the idea of making a decision to jump off if he can't hold on.. Dogs get proficient performing the DW day in day out and if they experience a bad uptake, they choose to stay on whatever it takes, ;-(.. Most DW falls or near falls result in pulled muscles in the effort to stay on..
This is why I keep walking Tracy's puppy Dani over the DW and the low AF and asking her to concentrate on the middle of the plank and walk steady, relaxed and straight. Shaping a deliberate consistent contact with the up contact is also very helpful in teaching a safe mount.
